Packing Slip

The packing slip is enclosed in a clear plastic envelope, which is then affixed to the outside of the shipping box. The following two graphics are examples of the current packing slip (top) and an older packing slip (bottom).

The Customer ID, Order number, and Serial number are found in the lower right corner of this packing slip.
